Senior Care Program - Caroline and Kent County

Senior Care

The Senior Care Program is a statewide program that provides case management and funds for services for people 65 or older who may be at risk of nursing home placement. Senior Care allows seniors to live with dignity and in the comfort of their own homes and at a lower cost than nursing facility care. Senior Care provides a comprehensive assessment of an individual’s needs, a case manager to secure and coordinate services, and a pool of gap filling funds to purchase services for individuals who meet program eligibility requirements. Services may include personal care, chore service, medications, medical supplies, adult day care, respite care, home delivered meals, transportation, and emergency response systems.

Coordination of Services

The Senior Care system of coordinated services is provided through the local area agency on aging, health department, and social services agencies. These services consist of screening, assessment, case management, and funds for gap filling services.

Screening and Referral

When seniors are referred for help, they are screened to determine if they may be eligible for Senior Care and other services. Screens may be performed in-kind by all participating agencies and include information about an individual’s age, income, assets and functional abilities. If the individual appears eligible for Senior Care, a referral is made for a full evaluation of need. Senior Care Assessment

Assessments are interviews conducted face-­to-face with the senior and his or her family members or significant others. The assessment is used to determine if the senior meets the established eligibility criteria and to develop a plan of care.

Upon completion of the assessment, a plan of care is developed that outlines services that the senior is currently receiving and those that are still needed. This plan of care and the assessment information are used to guide access to recommended services that the senior may receive. This service plan is based on the wishes of the senior and/or family, as well as the availability of services and funds. The case manager completes reassessments for eligibility and need every six months.

Case Management

Each Senior Care client is assigned a case manager. The case manager is responsible for assisting with the implementation of the care plan. This entails seeking out and ordering services, assisting the client or the client’s family to apply for other services, client advocacy and overseeing services that are provided.

Long Term Services and Supports Planning

The Community Options (CO) Waiver is designed to assist Maryland residents, aged 65+, or younger individuals with disabilities who require help with Activities of Daily Living (ADLs) such as bathing, dressing, and eating. The purpose of the waiver is to allow eligible individuals to receive long-term care services in their own homes or in assisted living settings, promoting independence and community living.

This program provides case management services for Medicaid eligible seniors to receive home or community-based services, which may include nurse monitoring, personal care, and emergency monitoring systems.

Contact Upper Shore Aging, 410-778-6000, and ask to speak to the MAP counselor for your county to obtain a Level One Screen, which is the first step in the application process for Community Options Waiver (COW), Community First Choice (CFC), or Community Personal Assistance Services (CPAS).

Meals on Wheels

Our Meals-On- Wheels program delivers meals to homebound seniors. Each week our volunteers deliver approximately 1,500 meals in the three counties we serve. In some cases, our volunteer is the only physical contact the senior has with the outside world.

Shopping for Seniors

For seniors living in Kent County, who are unable to shop for themselves, we offer a shopping service.  An Upper Shore Aging, Inc. staff person does the personal shopping and then delivers the groceries to the client’s home.
